Before you can be admitted into anyone of our establishments a needs assessment and risk assessment must be carried out.

What is a needs assessment?
A needs assessment gives us the opportunity to get to know you and understand your circumstances and assess your overall care needs.  As you play an essential role in the assessment it is important that we fully understand your views and wishes.

What is a risk assessment?
A risk assessment is carried out to predict behaviour or events that could cause harm, injury or impair well-being and thus take preventative or corrective actions to avoid the probability or their occurrence.  Risk assessments are about weighing up beneficial and harmful outcomes and stating their likely occurrence.  They play an essential role in ensuring we can appropriately meet your level of care or support needs.

Who will see the outcome of the assessments?
Information given on these forms will only be shared amongst those people who need to know about it.  All information will be kept confidential.

Will my placement be affected if I disclose information about previous problems?
No, all information disclosed will be assessed within context, unless we feel that you still pose a serious risk to yourself, other service users, staff or neighbours.

What happens next?
Should your application be successful you will be contacted and advised that we will be caring for you; we will put together a care package this is called a care plan and will detail a list of services you will receive.  Once agreed, a convenient date for you to move will be made. In the unlikely event that we are unable to offer you a placement you will be informed accordingly and if possible we will assist you in approaching a local service which may be able to assist you.
